A Case of Probable Reversible Cerebral Vasoconstriction Syndromes Identified by Serial Transcranial Doppler Study

Abstract

|
Reversible cerebral vasoconstriction syndromes (RCVS) are a group of disorders that have in common an acute presentation with headache, reversible vasoconstriction of cerebral arteries, with or without neurological signs and symptoms. A 49-year-old man presented recurrent episodes of severe headache and transient visual disturbance after sexual activity. Performed initial brain computed tomography angiography (CTA) was normal but brain Magnetic Resonance Angiography (MRA) showed segmental luminal stenosis in distal basilar artery after 2 weeks of headache onset. We performed serial transcranial Doppler (TCD) examination and showed an interval reversibility of vasoconstriction. After 14 weeks, follow-up TCD was done and showed a normalization of basilar artery flow velocity. In patients with RCVS, sequential TCD study might be a feasible non-invasive investigation.
